I actually do, i just dont label each section along with my comments, so some people may not notice I am. I have a form i fill out as I'm smoking, so everything is real time vs from memory, and each section is seperated with characteristics that get checked, and then turned into written comments.
Attached is the form for anyone interested in seeing it. Irs not pretty, but i think it covers everything, but I'm constantly adjusting things trying to improve it.
