I kept a book in Afghanistan, but I can't seem to find it. It had close to 50 bands in it, and I'm really mad that I have to rediscover most of that info.
Consider it a labor of love !!!!

I keep one. Picked up a small journal at walmart and fill it with notes and bands.

I kept one for a while, but I got too lazy to keep it up. It is a great resource when kept up. I should restart it. This what mine looks like. It is just a pocket journal I bought at the local store. I think a cheapy spiral notebook works best, IMHO.

I use humidornotes from time to time, usually more often than not especially when traveling around. But at least for me and this is in most cases, I get to caught up in the cigar smoking that I either forget to pull out my humidornotes or get too lazy. Thankfully, I tend to be a creature of habit so I usually remember the cigars I do and don't like so just gets tough filling up something more than my usual cigar rotation, if I don't like the cigar, it will be a while before I try it again let alone do a mini-review on it.
